Supported Living Services Designed Around Each Individual

SERVICES

Cal4Autism SLS provides individualized Supported Living Services to adults with developmental disabilities who want to live in a home of their choice and participate meaningfully in their communities. Services are based on each person’s assessed needs, preferences, goals, Individual Program Plan, living arrangement, and Regional Center authorization.

Daily Living and Household Support

Support may include:

• Meal planning
• Grocery shopping
• Cooking
• Cleaning
• Laundry
• Personal routines
• Household organization
• Scheduling and time management
• Budgeting and money-management skills
• Maintaining a safe and comfortable home
• Communication with landlords or utility providers when authorized
• Learning to use household appliances safely
• Developing routines that support independence

Community Participation and Integration

Support may include:

• Recreation
• Social activities
• Community events
• Volunteering
• Employment-related routines when applicable
• Shopping and errands
• Public transportation training
• Building natural supports and relationships
• Exploring hobbies and personal interests
• Cultural and spiritual activities chosen by the individual
• Community safety
• Appointment transportation or coordination when authorized

Person-Centered Planning and Service Coordination

Support may include:

• Individualized service planning
• Participation in the IPP process
• Coordination with Regional Center service coordinators
• Collaboration with authorized family members and natural supports
• Communication with healthcare and behavioral providers when authorized
• Review of changing needs and goals
• Support during service transitions
• Coordination of authorized services and staffing schedules
• Documentation of progress toward identified goals

Housing and Living Arrangement Support

Cal4Autism SLS may assist individuals with exploring and facilitating living arrangements through local relationships with landlords and property managers. Support may include identifying possible housing options, coordinating housing-related communication, assisting with transitions, and promoting long-term housing stability.

Health and Wellness Coordination

Support may include:

• Scheduling medical and dental appointments
• Attending appointments
• Helping individuals prepare questions for providers
• Supporting communication with healthcare professionals when authorized
• Medication assistance and documentation as authorized
• Supporting wellness routines
• Nutrition and physical-activity goals
• Emergency preparedness
• Following authorized health and safety plans
• Coordinating with approved members of the support team

Skill Development

Support may include:

• Communication
• Decision-making
• Problem-solving
• Self-advocacy
• Safety awareness
• Personal organization
• Time management
• Technology use
• Appointment preparation
• Household skills
• Budgeting skills
• Confidence-building
• Practicing new skills at the person’s preferred pace

Flexible Staffing and Support

Cal4Autism SLS develops staffing schedules based on each individual’s assessed needs, authorized service hours, preferences, health and safety considerations, and Individual Program Plan. Support may range from scheduled weekly assistance to services available up to 24 hours per day when authorized.

Choice and Control

Individuals are supported in making decisions about their home, daily routines, relationships, activities, services, and personal goals.

Self-Advocacy

Individuals are supported in expressing concerns, asking questions, communicating preferences, understanding their rights, and participating in decisions.

Active IPP Participation

Each person is encouraged and supported to participate meaningfully in their Individual Program Plan process.

Support That Evolves

Services are reviewed and adjusted through the appropriate planning and authorization process as the individual’s needs, preferences, circumstances, and goals change.

Dignity and Respect

Services are provided in a manner that protects privacy, promotes dignity, and respects each person’s identity, culture, communication style, relationships, and preferences.
